How to Request Sick LeaveHZZO
Except for parents of sick children, sick leave cannot be opened via email without a prior examination or consultation with the doctor.
In that case, please book an appointment with the doctor or come during acute care hours.
If your condition prevents you from coming to the office, you can request a phone consultation. In special cases, when medically justified, our team will make a home visit.
Sick leave for child care
When you have a confirmation or excuse note from the pediatrician for your sick child, request parental sick leave online. The doctor will review your request and send you a sick leave certificate within 24 hours to your email address.
Instructions for submitting a request for sick leave for child care
Visit the request submission website
Fill in the required fields with your information and your child's information
Submit the form and check your email
After we receive your form, our system will send you an automatic confirmation message to the email address you provided. The automatic message usually arrives within seconds after you submit the form.
If you don't see the message in your inbox after about ten minutes, please check if the message is in your spam folder. If you find it there, be sure to move it to your inbox and/or confirm in your email program that the message is not SPAM. This will ensure successful receipt of future messages, including your sick leave certificates.
Wait for the certificates
We will send the certificates to your email address within 24 hours. If we need additional explanations or documentation, the doctor will contact you directly via email or phone call.
